Contact form
Capture leads straight from your profile page — no app, no paper.
The Contact form block lets a visitor leave their details from your profile page. Each submission becomes a contact, saved with its source.
Adding the form
- Open the page editor.
- Drag the Contact form block where you want it.
- Choose the fields and the actions on submission (see below).
- Save the template.
A screenshot showing the form as it appears on mobile helps your users know what to expect.
Fields
- Name, email, phone, message (always available).
- Custom fields (depending on your plan / module).
Actions on submission
In the editor, you choose what happens when the form is sent:
- Create a contact — the submission is saved to your contacts (source: profile form).
- Email notification — an email lets you know (with the details).
- Webhook — a call is sent to the URL of your choice (automation). See Webhooks.
- Download a vCard — the visitor can also grab your contact.
These actions combine: for example, create a contact and get an email notification and call a webhook.
Example journey
- A prospect taps your NFC card: your profile page opens.
- They fill in the form (name, email, message) and send it.
- The submission creates a contact (source: profile form) and you get an email notification.
- You find the lead in Contacts, ready to follow up or export to your CRM.
Anti-spam
The form is protected against abusive submissions (a limit on submissions per minute and per IP address).
Good to know
- Find all submissions in Contacts, filterable by source and date. See Contacts.
- The availability of custom fields and some actions (webhook, CRM export) depends on your plan. See Subscription.
Troubleshooting
- I'm not receiving email notifications: check your spam folder and that the Email notification action is enabled on the block.
- A submission doesn't show up in Contacts: make sure the Create a contact action is enabled. See Contacts and Troubleshooting.
